Standard lipid panels report LDL, HDL, and triglycerides, but ApoB is increasingly recognized as one of the most important markers for cardiovascular risk. Understanding your ApoB result and treatment targets can help you have a more informed conversation with your healthcare provider about heart disease prevention.
ApoB, or apolipoprotein B, is a protein found on every atherogenic lipoprotein particle in your blood—LDL, VLDL, IDL, and Lp(a). Because each particle carries exactly one ApoB molecule, your ApoB level reflects how many plaque-forming particles are circulating, not just the cholesterol mass inside them. Think of LDL cholesterol as measuring cargo weight inside trucks, while ApoB counts how many trucks are on the road. Two people with identical LDL cholesterol can have very different ApoB if one has many small, cholesterol-depleted LDL particles and the other has fewer, larger particles.
Higher ApoB means more particles that can enter artery walls and drive atherosclerosis—the buildup of plaque that leads to heart attacks and strokes. In many studies, ApoB outperforms LDL cholesterol for predicting cardiovascular events, especially when triglycerides are elevated, in metabolic syndrome, type 2 diabetes, or in patients on statins with residual risk despite controlled LDL.
ApoB is reported in mg/dL or g/L on lab reports. Conversion is straightforward: 1 g/L equals 100 mg/dL. For example, 0.85 g/L equals 85 mg/dL. ApoB can be measured fasting or non-fasting, unlike triglyceride-dependent LDL calculations. Many clinicians order ApoB with a standard lipid panel, though you may need to request it specifically—it is not included in basic lipid panels at all labs.
Clinical ApoB categories help interpret your result. Optimal ApoB is below 60 mg/dL (below 0.60 g/L)—maintain lifestyle habits and routine screening. Desirable is 60 to 79 mg/dL—favorable particle burden. Near-optimal is 80 to 99 mg/dL—below the 100 mg/dL target for many adults. Borderline high is 100 to 119 mg/dL—lifestyle changes and cardiovascular risk review are warranted. High is 120 to 139 mg/dL—medical review and lipid-lowering therapy should be considered. Very high is 140 mg/dL or above—aggressive treatment and screening for familial hypercholesterolemia may be needed.
Treatment targets depend on your overall cardiovascular risk profile, not just the category label. For adults at general or low-to-moderate risk in primary prevention, an ApoB target below 100 mg/dL (below 1.00 g/L) is commonly used. For elevated-risk individuals—such as those with diabetes, smoking, strong family history, high triglycerides, or metabolic syndrome—the target is often below 80 mg/dL (below 0.80 g/L). For very high-risk patients—including those with prior heart attack, stroke, established ASCVD, or familial hypercholesterolemia with events—the target may be below 65 mg/dL (below 0.65 g/L). A borderline ApoB of 95 mg/dL may be acceptable in a low-risk young adult but above target for someone with prior heart attack. Classification describes your current level; treatment targets describe where you should aim based on your full risk picture. Always interpret ApoB alongside blood pressure, glucose, smoking status, family history, and overall ASCVD risk scores.
One of the most important concepts in ApoB interpretation is discordance with LDL cholesterol. Discordance occurs when ApoB is elevated relative to LDL-C—your particle count is higher than your LDL cholesterol suggests. This is flagged when ApoB is at least 30 mg/dL above LDL, or when ApoB is 100 mg/dL or higher with LDL below 100 mg/dL. Discordance is common with insulin resistance, high triglycerides, and small dense LDL particles. It is especially relevant in South Asian populations, where atherogenic dyslipidemia often shows normal LDL, low HDL, elevated triglycerides, and high ApoB relative to LDL.
Can ApoB be high with normal LDL? Yes—and this is one of the main reasons to test ApoB. High triglycerides increase VLDL and small dense LDL particles, raising ApoB while LDL-C stays in the normal range. Standard Friedewald LDL calculations can miss this hidden particle risk. If your LDL looks reassuring but metabolic risk factors are present, ApoB testing can reveal atherogenic burden that LDL alone does not capture.
ApoB is related to but different from non-HDL cholesterol. Non-HDL equals total cholesterol minus HDL and estimates cholesterol in all apoB-containing particles. ApoB counts the particles themselves. Both predict cardiovascular events well. ApoB is more precise when particle number and cholesterol content diverge. Non-HDL is widely available without extra testing; ApoB requires a specific assay. When ApoB exceeds non-HDL by more than about 10 to 15 mg/dL, you likely have cholesterol-depleted, small dense LDL particles.
Consider ApoB testing when LDL appears normal but triglycerides are high, when metabolic syndrome or insulin resistance is present, when there is family history of early heart disease, when LDL and non-HDL disagree, when residual risk persists on statins, or when South Asian atherogenic dyslipidemia is suspected. ApoB is not needed for everyone—standard lipid panels suffice for many low-risk adults with concordant results.
Evidence-based approaches to lower ApoB include a Mediterranean-style diet low in saturated fat, soluble fiber from oats and legumes, weight loss if overweight, and at least 150 minutes of exercise per week. Statins lower ApoB by 30 to 50 percent; ezetimibe adds 15 to 20 percent; PCSK9 inhibitors can reduce ApoB by 50 to 60 percent. Plant sterols, omega-3 for high triglycerides, and treating insulin resistance also help. Recheck ApoB six to twelve weeks after therapy changes, then every six to twelve months once stable.
Residual cardiovascular risk is ongoing heart attack and stroke risk despite treatment—often because ApoB remains elevated even when LDL-C is controlled. Up to 30 to 40 percent of statin-treated patients have ApoB above target. If you reach LDL goals but ApoB stays high, discuss therapy intensification with your doctor—switching to high-intensity statin, adding ezetimibe, or using PCSK9 inhibitors may be appropriate.
ApoB is not the same as Lp(a). ApoB is on all atherogenic particles. Lp(a) is a specific genetically determined lipoprotein measured with a separate test. High Lp(a) adds to total ApoB but requires dedicated testing to identify. Both are independent cardiovascular risk factors worth discussing with your clinician.
Age and sex influence population comparisons. U.S. men have a higher median ApoB than women, and ApoB tends to rise with age. The same value means something different for a 30-year-old versus a 65-year-old. Population percentiles provide educational context but are not substitutes for clinical targets—use ApoB category and risk-based treatment goals for medical decisions.
